Findings
On or about August 11, 2024 and August 31, 2024, the Alleged Victim (AV) and Witness #4 (W4) engaged in altercations. No injuries were noted, however, AV requested to be sent to the hospital. AV was diagnosed with a contusion to his/her head. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
