Findings
The Alleged Victim (AV) and Witness #1 (W1) engaged in multiple altercations between July 2024 and October 2024. W1 is care planned to have safety checks six times per shift and AV every 2-3 hours. AV and W1 were to be kept separated and to monitor W1 for increased agitation, aggression and emotional outbursts. The facility's failure to ensure resident safety and ensure care plans are being followed is a violation of resident rights, is considered neglect of care and constitutes abuse.
