Findings
The Alleged Victim (AV) is care planned for stand by assistance with toileting. On or about September 2, 2024, AV was found by staff, during a med pass, on the toilet, crying and struggling to change his/her brief. Alleged Perpetrator #2 (AP2) was seen leaving AV's room and had advised another staff of a skin issue on AV a short time before AV was found unattended. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure to ensure the care plan is being followed is a violation of Oregon Administrative Rules.
