Findings
The Alleged Victim (AV) relies on the facility to provide basic care and coordination of services. According to an investigation, on or about August 1, 2024, AV's lab results were faxed to AV's physician indicating further treatment was needed. The facility failed to follow up with AV's physician to coordinate care for AV, which is a violation of resident rights and is considered neglect, which constitutes abuse.
