Findings
On or about September 9, 2024, the Alleged Victim (AV) told staff that Alleged Perpetrator #2 (AP2) took his/her phone to replace a battery and never returned it. AP2's actions are a violation of resident rights, are considered neglect of care and constitute financial abuse. The facility's failure is a violation of Oregon Administrative Rules.
