Findings
Alleged Victim (AV) has a known history of falls. On or about September 16, 2024, AV had dark purple bruising on the left side of AV’s face and forehead. The facility had no incident report or documentation to explain the injury AV had sustained. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
