Findings
The Alleged Victim (AV) is care planned as a two person transfer. AV has fallen previously while being assisted by Alleged Perpetrator #2 (AP2). On or about September 24, 2024, AP2 was transferring AV alone, and trying to pull up AV's pants when AV became weak and AP2 had to assist AV to the ground. AV was not injured, however, AV did complain of pain after the incident. AP2's actions of not following AV's care plan is a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure to ensure AV's care plan was followed is a violation of Oregon Administrative Rules.
