Findings
The Alleged Victim (AV) has had multiple unwitnessed falls. AV has suffered approximately 18 falls within the last 5 months. Three of AV's falls resulted in significant injury and change of condition. The results of AV's falls included 21 stitches on his/her lower left leg, a large bruise to AV's back and showing signs of confusion, and dizziness and shortness of breath, in which AV was sent to the local hospital. The facility's failure to ensure AV's safety is a violation of resident rights, is considered neglect of care and constitutes abuse.
