Findings
The Alleged Victim (AV) relies on the facility for his/her needs. According to an investigation, on or about October 4, 2024, AV gained access to a secure kitchen area through a broken kitchen door, where he/she grabbed a knife and threatened staff. The failure placed AV at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
