Findings
The facility failed and Alleged Perpetrator 2 (AP2) failed to provide appropriate services according to the Alleged Victim (AV)’s needs, resulting in AVs loss of dignity. AV needs reminders for mealtimes and is to be offered a snack and drink twice daily outside of meals. AV needs toileting assistance at the beginning, middle, and end of each shift, before and after meals. On or about September 22, 2024, Witness 3 (W3) arrived at facility and found AV lying in bed, soaked with urine. Witness 2 (W2) investigated this incident and determined that AP2 had not, changed AV all shift, and had not given AV breakfast or lunch on September 22, 2024. AV's progress notes indicate AV had redness and pain to AV's peri area since approximately July 24, 2024 and no interventions in place for skin issues that needed daily care, no oversight or hands-on training for AP2 regarding refusal of care, and refusal of meals. The facility and AP2 failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
