Findings
The facility failed to appropriately care plan and implement interventions for the Alleged Victim (AV) to reduce the risk of falls. Over the course of two months, AV suffered multiple falls, with and without injury, leading to repeated unreasonable discomfort and pain. The facility failure is a violation of resident rights, are considered neglect of care and constitute abuse.
