Findings
Alleged Victim (AV) has a known history of falls. On or about July 2024, AV sustained three (3) falls, on or about August 2024, AV sustained six (6) falls, on or about September 28 to October 5, 2024, AV sustained four (4) falls. On or about October 13, 2024, at approximately 8:30PM, AV was found on the floor of his/her bathroom and was unable to move due to pain in his/her shoulder and head: AV was transported to the ER and was diagnosed with a closed fracture of his/her acromial process of the right scapula. The facility failed to properly care plan and mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
