- Alleged Victim (AV) relies on the facility for his/her care needs. Witness 4 (W4) says AV had lost a lot of weight when W4 went to see AV, AV's teeth were hurting, and AV couldn't eat. W4 told the facility that AV needed to go to the dentist to get bottom dentures. AV said that facility was neglecting AV and not accommodating AV's food, giving AV hard/ crunchy food. AV says the pain is so bad, that AV can't eat what the facility provides, and AV reports he/she is "starving". When looking at AV's weight charting, it appears that the facility failed to properly weigh AV monthly. AV was weighed in August at 213.1 lbs., was not weighed in September and then weighed twice in October at 198.9 lb. & 197.8 lb. resulting in a 15.1-pound loss. No weight loss tracking, change of conditions or interventions were put into place for AV until after APS entered the facility on or about 10/17/2024 despite the facilities own policy requiring them to do so. The facility failed to provide service by failing to administer basic care needs around tooth pain and monitoring wight loss, which is a violation of resident rights is neglect of care and constitute abuse.
