Findings
Alleged Victim (AV) relies on the facility for his/her care and requires assistance with transfers. On or about July 22, 2024, the Alleged Perpetrator #2 (AP2) assisted AV with a transfer resulting in AV hitting their head on the wall. During the investigation it was discovered that AP2 was partially trained on transfers and AVs care plan was inconsistent regarding AV's care needs. The allegation that AP2 failed to provide basic care and services necessary for AV health and safety was investigated and wrongdoing was unable to be determined. The facility failed to provide appropriate services according to Alleged Victim’s needs, relating to care planning and providing fully trained staff, which is a violation of resident rights, is neglect of care which constitutes abuse.
