Findings
The Alleged Victim (AV) relies on the facility for his/her care and has a dementia diagnosis. According to an investigation, AV pulled their catheter out on or about October 3, 2024 and was sent to the hospital for treatment, where a new catheter was placed. AV's care plan was updated to remind AV not to pull the catheter out but lacked clear instruction on how and when to issue reminders. Then, on or about October 7, 2024, AV, once again pulled their catheter out resulting in AV being sent back to the hospital for treatment. The facility failed to properly care plan resulting in repeated unreasonable discomfort, which is a violation of resident rights, is neglect of care which constitutes abuse.
