- Witness 1 (W1) and Alleged Victim (AV) have a known history of being involved in other resident-to-resident altercations. On or about August 20, 2024, AV bumped into W1 in the dining room when AV was self-propelling AV’s wheelchair. W1 then hit AV in the head three (3) times. As a result, W1 broke AV’s grasses. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
