Findings
The Alleged Victim (AV) relies on the facility for his/her care. Witness 3 (W3) has a history of behaviors and physical altercations. According to an investigation, on or about October 21, 2024, there was an unwitnessed physical altercation where W3 hit AV in the head/face causing AV to cry. The facility failed to provide services to keep AV safe, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
