Findings
On or about October 5, 2024, Alleged Perpetrator #2 (AP2) was attempting to take the Alleged Victim's (AV) vitals when AV became upset and aggressive. Witness #3 (W3) responded to assist AP2 and heard AP2 call AV "crazy" multiple times, causing AV to become more upset. AP2's actions are a violation of resident rights, are considered neglect of care and constitute verbal abuse. The facility's failure to protect AV from verbal abuse is a violation of Oregon Administrative Rules.
