Findings
The Alleged Victim (AV) relies on the facility for their care. According to an investigation, on or about November 7, 2024, Alleged Perpetrator 2 (AP2) was attempting to engage the brake of AV's wheelchair from the opposite side by reaching across AV's chair. After engaging the brake, AP2 stood up, and AP2's elbow made contact with AV's nose, causing AV's nose to begin bleeding and eyes to water. AP2’s actions are a violation of resident rights, are considered neglect of care and constitute abuse. Prior to this incident, AP2 was scheduled for additional training and/or oversight, however, there is no documentation this was done. The facility failed to provide a safe environment which is a violation of resident rights, is neglect of care and constitutes abuse.
