Findings
On or about November 3 and November 4, the Alleged Victim (AV) suffered falls, resulting in injuries to his/her head and a skin tear on the right elbow. AV suffered 7 falls between September 23, 2024 and November 8, 2024, 3 of which resulted in injury. The facility failed to properly care plan for the reduction of AV's falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
