Findings
The Alleged Victim (AV) relies on the facility for his/her care, and is care planned for assistance with transfers. On or about August 4, 2024, the Alleged Perpetrator 2 (AP2) was attempting to transfer AV, when AV and AP2 fell to the floor. Despite AV being in pain after the fall, AV was not sent to the hospital until August 9, 2024, where AV was diagnosed with a back fracture. The facility failed to provide services, to include responses to AV's change of condition and monitoring and to ensure that timely medical treatment is provided, which is a violation of resident’s rights is neglect of care and constitutes abuse.
