- The Alleged Victim (AV) relies on the facility to provide all medications and treatments. The AV has a history or skin issues and is prescribed as needed treatments. According to an investigation, on or about December 2, 2024, the AV was identified to have significant worsening of the skin condition. The AV expressed discomfort, itching, and burning due to the worsening skin condition. The medicinal treatments for the AV's skin condition were not provided by the facility in November or December of 2024. The facility failed to provide AV’s medication as ordered, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
