Findings
The Alleged Victim (AV) relied on the facility for care. According to an investigation, on or about October 25, 2024, in AV's room there was fecal matter on bathroom surfaces, AV's commode was not cleaned out, AV's bedding was soiled, garbage and laundry was not removed, and there were paper towels placed over AV's dog's waste on the floor in AV's room. The facility failed to provide appropriate services, to include following AV's care plan and/or the implementation of effective interventions and resident monitoring, resulting in unreasonable discomfort and a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse as defined by OAR 411-020-0002(1)(b)(A)(i).
