- Alleged Victim (AV) relies on the facility to provide a safe and secure environment. According to an investigation, on or about December 14, 2024, Alleged Perpetrator 2 (AP2) mistook the AV for a facility visitor, opened the secured door, and provided the AV with the door code, resulting in the AV eloping and being exposed to potential harm. The AV was picked up by a community member approximately half an hour later walking along a highway about three quarters of a mile from the facility. AP2 actions are considered neglect and constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of resident rights, is neglect of care, and constitutes abuse.
