- According to the documentation, the facility failed to properly care plan appropriate interventions for the Alleged Victim’s (AV) exit seeking behavior leading to multiple elopements from the secured memory care facility. On or about December 14, 2024, the AV eloped from the secured memory care approximately three times. The AV was able to obtain the code to exit the facility. The AV used the code to leave the facility approximately three times in one day. Due to AV’s cognitive status, they were not able to find their way back to the facility and was found in the parking lot of the assisted living. The AV was able to be redirected back to the secured unit after each elopement. The facility failed to implement appropriate interventions after the initial elopement to ensure the AV was not able to leave the secured unit unaccompanied, placing them at risk for harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
