Findings
The Alleged Victim (AV) relies on the facility to assist with bathing and hygiene. The AV is care planned for showers twice weekly. According to an investigation, on or about the weeks of November 11, 2024 and December 4, 2024, the facility failed to provide assistance to the AV for showers twice weekly, causing discomfort. The facility failed to provide the AV proper hygiene, which is a violation of resident’s rights, is considered neglect of care, and constitutes abuse.
