Findings
On or about December 24, 2024, the Alleged Victim (AV) pressed his/her call light for assistance. No staff members answered AV's call light. AV tried to transfer him/herself to the restroom and slid off his/her wheelchair to the floor. A staff member that was working was found sleeping on shift and did not answer AV's call for assistance. AV was not injured, however, AV was placed at risk for harm. The facility's failure to ensure they have qualified awake caregivers to answer call lights timely is a violation of Oregon Administrative Rules.
