Findings
The Alleged Victim (AV) has history of elopement and exit seeking behaviors. According to an investigation, on or about December 26 2024, the AV eloped and was found by law enforcement three miles from the facility, placing the AV at risk for serious harm. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care, and constitutes abuse.
