Findings
On or about January 1, 2025, Witness #1 (W1) was trying to hit Witness #6 (W6) with his/her walker. The Alleged Victim (AV) tried to step in to stop W1 and W1 punched AV. W1 and AV have had past altercations, no interventions were put into place to ensure no further altercations would occur between the two. The facility's failure to properly care plan regarding interventions is a violation of resident rights, is considered neglect of care and constitutes abuse.
