Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about December 16, 2024, AV was in (other resident’s) room but was not actively causing any problems and residents were sitting in the room together without issue. Alleged Perpetrator 2 (AP2) approached AV and other residents and instructed them to leave the room, and this upset AV and other residents, leading to AV approaching AP2 in a upset manner. AP2 was witnessed throwing a (communication device) at AV, causing an injury to AV’s head. AP2 failed to protect AV from physical abuse, which is neglect of care and constitutes abuse. The facility failed to protect AV from physical abuse, which is a violation of Oregon Administrative Rules.
