Findings
The Alleged Victim (AV) relies on the facility for their care. W1 has a history of approaching AV and engaging behaviors not welcomed by AV. According to an investigation, on or about February 15, 2025, W1 kissed AV on the lips. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care which constitutes abuse.
