- The Alleged Victim (AV) relies on the facility for his or her safety. AV wanders around the facility and has a history of exit seeking. AV service plan indicates, in the event of an emergency, staff should prioritize AV's safety due to AV's tendency to exit seek. AV requires consistent guidance and encouragement during evacuation procedures. AV should be escorted to the designated safe area by staff who can provide reassurance and support throughout the process. On or about January 17, 2025 AV eloped from the facility unit during a fire drill, as staff were helping other residents. The facility failed to provide a safe environment, and follow Av’s service plan, with known wandering and exit seeking behaviors, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
