Findings
The Alleged Victim (AV) relies on the facility to provide appropriate care and services. The AV has a history of falls, a history of not using their call light, as well as a health issue that causes AV to become dizzy at times. According to an investigation, between December 30, 2024, and January 28, 2025, AV suffered three falls, resulting in multiple injuries, including back pain, head injury and discoloration to his/her wrists and forearm. The facility failed to ensure the care plan was followed and/or implement interventions to mitigate AV’s increasing fall risk, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
