- The Alleged Victim (AV) relies on the facility for assistance with activities of daily living. According to an investigation, on or about January 28, 2024, the AV reported the Alleged Perpetrator 2 (AP2) was rough when providing care. Bruising was identified on the AV’s right wrist and a reddened area noted on the left hip. AP2 did not received training for AV's care needs. The facility failed to ensure the training of all staff to prevent rough treatment, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
