Findings
Alleged Victim (AV) relies on the facility for his/her care needs. AV experienced at least nine (9) unwitnessed falls in the period from December 10yth, 2024, through February 5th, 2025. Despite AV’s repeated falls, AV was not service e planned for any safety checks until AV’s service plan was updated. As a result of the falls, AV experienced a laceration to their elbow on or about January 31, 2025, and February 3, 2025, and AV experienced a cut to his/her head on or about February 5, 2025. The facility failed to adequately care plan around AV’s known history of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
