- On or about February 5, 2025, the facility failed to provide appropriate supervision to the Alleged Victim (AV) according to his/her care needs which resulted in AV eloping the facility. At time of incident, it was dark, cold and icy outside. AV was returned to the facility by an unknown community member who discovered AV 1 mile away from the facility. AV was returned looking cold and exhausted as result of the incident. Based on facility documentation and interviews, AV has been diagnosed with a cognitive disease and is unable to maintain his/her health and safety independently. AV is considered fit and is able outwalk most staff. AV had a prior elopement incident on January 14, 2025. The facility's failure to implement effective and appropriate elopement interventions resulted in AV leaving the facility unattended, causing AV unreasonable discomfort due to the cold elements of the early morning winter weather and placing AV at risk of moderate harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
