Findings
The Alleged Victim (AV) relies on the facility for care and to provide a safe environment. According to an investigation, on or about February 9, 2025, AV and Witness 1 (W1) were both in the common area, when W1 touched AV on the arm and AV proceeded to punch W1 three times in the arm. W1 responded by trying to bite AV. Both residents have a history of resident-to-resident altercations. The facility failed to provide adequate interventions for AV and W1, which is neglect and constitutes abuse.
