- Based on witness statements, investigator observations, and documentary evidence collected during Adult Protective Services (APS) Investigation #00383344, the APS Investigator determined that on or about February 9, 2025, the facility failed to provide adequate oversight of the facility's Medication Administration System. This facility failure resulted in the diversion of two tablets from the Alleged Victim’s (AV) prescribed narcotic pain medication supply. The medication was taken by an unknown individual, referred to as Alleged Perpetrator 3 (AP3). The theft of AV’s prescribed narcotic pain medication by AP3 is considered financial exploitation, which constitutes abuse. The facility failed provide a safe Medication Administration System for AV which resulted in the loss of AV's property. The facility did not ensure accurate recordkeeping or secure handling of AV's narcotic pain medication, which is considered a violation of resident rights, is considered neglect of care and constitutes abuse.
