Findings
The Alleged Victim (AV) and Witness #2 (W2) were known to have altercations, and staff were instructed to keep eyes on the two at all times and keep them separated. On or about February 21, 2025, W2 was arguing with AV and then punched AV in the left side of his/her ear. There was not sufficient staff in the cottage at the time of this incident to prevent the altercation. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
