Findings
The Alleged Victim (AV) relies on the facility to provide care and services. According to an investigation, the AV had a change of condition and had suffered multiple falls, and the facility did not update the service plan to include interventions for the change. The facility failed to update AV's service plan when AV had a change of condition, resulting in AV falling and sustaining injuries. This is a violation of resident rights, constitutes neglect which is abuse.
