Findings
The Alleged Victim (AV) is dependent on the facility to meet his/her safety needs and has a history of falls. According to an investigation, on or about September 9, 2024, AV was heard falling and was found on the floor on the weighing scale in the common area. AV's service plan, dated July 28, 2024, instructed staff to ensure hallways were kept clutter free and provide frequent visual checks to prevent falls. There was no documentation of the visual checks, and AV tripped over the scale, resulting in injury and bruising to the shoulder, chest and hip that appeared on September 12, 2025. The facility failed to follow the care plan for AV, which is a violation of resident rights, is considered neglect of care which constitutes abuse.
