- The Alleged Victim (AV) was a known fall risk with a history of falls. The facility failed to care plan according to the AV's needs, change of condition, and number of falls. According to an investigation, the AV experienced multiple unwitnessed falls between January 17, 2025, and March 5, 2025, resulting in abrasions, pain, and discomfort. The failure is a violation of resident rights, is considered neglect of care, and constitutes abuse.
