Findings
The Alleged Victim (AV) relies on the facility for their care and has a history of falls. According to an investigation, on or about January 23, 2025, AV experienced an unwitnessed fall, resulting in pain and a bruise. The facility failed to appropriately plan care, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
