- Adult Protective Services (APS) case #00389939 was opened for Investigation by the Department on March 19, 2025, in which it was determined that the facility failed to provide appropriate services according to the Alleged Victim's (AV) personal hygiene care needs. Based on facility documentation, ten weeks prior to the investigation, AV was only provided one shower per week instead of two. Four of the ten weeks, AV's showers were not documented and could not be verified as being done according to AV's care plan. Based on interviews, AV expressed discomfort as a result of not receiving proper hygiene care due to AV having open sores in his/her peri area which AV is unable to completely care for without staff assistance. The facility's failure resulted in AV’s condition worsening causing further unreasonable discomfort, which is a moderate violation of resident rights, is considered neglect of care and constitutes abuse.
