Findings
Alleged Victim (AV) relies on the facility for his/her care. AV is care planned to have line of sight supervision from facility care staff. On or about March 23, 2025, AV had an unwitnessed fall, resulting in right foot toe fracture and unreasonable discomfort. According to an investigation, facility staff were helping other residents and did not have AV in line of sight. The facility failed to follow the care plan which is a violation of resident’s rights is neglect of care and constitutes abuse.
