Findings
On or about March 27, 2025, Witness #1 (W1) hit the Alleged Victim (AV), causing unreasonable discomfort. AV and W1 rely on the facility for redirection and ensuring their safety. Repsondent failed to provide a safe environment for AV. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
