Findings
On or about April 4, 2025, Alleged Victim (AV) was not given AV's evening dose of one his/her seizure medications. Av did not have any adverse reactions to the medication error; however, the facility did fail to provide a safe medication administration system which is a violation of Oregon Administrative Rules.
