Findings
The Alleged Victim (AV) had suffered falls on April 5, 2025, and April 10, 2025. On April 10, 2025, fall, AV suffered a broken hip and upon his/her return from the hospital, AV was unable to ambulate without staff intervention. AV repeatedly tried to walk without staff assistance. On or about April 11, 2025, AV suffered another fall and was in a lot of pain. AV was sent back out to the hospital and diagnosed with a fractured rib. Respondent failed to implement meaningful interventions to reduce AV's risk of falls. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
