Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about April 15, 2025, AV was agitated and threw water on the Alleged Perpetrator 2 (AP2). According to an investigation, AP2 proceeded to fill up a cup of water and threaten AV with throwing the water at them, causing AV emotional distress and loss of dignity. AP2 actions are considered verbal/emotional abuse. The facility failed to protect AV from verbal/emotional abuse which is a violation of Oregon Administrative Rules.
