Findings
On or about March 16, 2025, and April 12, 2025, The Alleged Victim (AV) and Witness #1 (W1) engaged in an altercation, which resulted in pain to AV. AV has a history of disruptive behaviors, and staff are to redirect or remove AV from situations that would cause altercations. W1 is also to be redirected to avoid altercations. The facility failed to ensure the safety of these residents, resulting in altercations. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
